Find the Invitation Letter, Student Application and Student Musical Activities Listunderneath the list of the County in which your school is located.
Open the file, type in the appropriate information and print the documents. Remember to secure the required signatures on the application form. You may see a box at the top of the page which asks you to "Enable Editing" Click on that box to activate the fillable WORD documents.
Send your completed hard copies of the Application and Student Musical Activities List to the address listed at the bottom of the application. You may print the application and activities list back to back if you wish. Make sure your name is on both the application and activities list.
